First, stabilize.

Also, sometimes you need to spread your expertise thin; the volunteer EMS where I grew up was mostly EMTs with one paramedic carried by the fire department.

Here in California, we have CalFire, which has EMTs or paramedics with each fire truck and then special rescue or support vehicles for certain calls.

The actual transporting ambulance isn't expected to do any of the patient stabilization or extrication. They just take a stable patient from the first responders to the hospital (or to a helicopter which is led in by CalFire as well).

Different departments have different compositions of volunteers and professional - ours is a rural town station, so it's volunteer, but even more rural stations are professional. (While actual urban stations are municipal and not CalFire and often actual officers.)
