Software changes put different stresses upon electronics or apply new requirements or tolerances. That can reveal parts that were faulty or out of spec that weren't noticed before.

For instance, if the memory in a subprocessor was bad, but only used when updating firmware, you'd not notice until it tried to write there and then read back bad data.
