Towing issue has now happened on 2 separate occasions.

First time, After towing the trailer I unhitch, get in truck and put seatbelt on, depress brake and push the accelerator it backs into the trailer even though I swore it went into drive. I figure I was tired and didn’t pay attention and it went into reverse.

Yesterday I unhitch the trailer, put it in drive myself cause I just had to roll forward to put the lock on. The truck hits the trailer, I think the truck raised and caught the trailer, I get out nope not connected. I get in put it in drive and again it goes back and hits the trailer, this time I realize what’s happening. Finally on the third try it went forward.

Has this happened to anyone else, any idea what’s happening? Truck has 5k miles on it.

Was the truck in trailer mode without being hitched to the trailer, both times?

It’s not likely, but was the trailer’s light harness still connected to the truck?

Harness was not plugged in, yes truck was in tow mode, unless it automatically turns it off when truck is disconnected, I’ve never paid attention, but don’t recall having to turn off tow mode after disconnecting.

The owners manual says:

“Trailer Mode
Trailer Mode must always be active when towing a trailer. When you connect a trailer's electrical connection (see Electrical Connections), Cybertruck automatically engages Trailer Mode. Trailer Mode disengages when you disconnect the trailer's electrical connection. To engage or exit Trailer Mode manually, touch Controls > Towing & Hauling > Trailer Mode on the touchscreen. One of the following indicators displays on the touchscreen:
Icon of a blue trailer.
Trailer Mode is active.”

And also says:

“Warning
Do not rely on Cybertruck to automatically detect a trailer in all cases. Always check that Trailer Mode is engaged before towing a trailer.”

This probably too far out of the box, but I am just wondering if, when trailer mode is engaged and the truck is first driven if the truck measures the trailer mass by attempting to move slightly, even if there is no trailer attached.

None of that would cause the truck to reverse when it is showing drive on the display.

I had some similar issues as well as some screen warnings. The truck stayed in "tow mode" perpetually, but also said features wouldn't work. Turns out a connector was loose. Quick fix at the service center.

The problem is the truck showed drive but it went backwards several times.

I park in front of my trailer a lot without hitching. If I hit the brake to have it auto switch into drive it usually puts it in reverse and I have to go move it into drive (maybe it's the ground in front of it being different and the trailer being black). I thought it would see the trailer behind me, but it consistently does that. I have never put it in drive and had it go backwards though.
