I’m going on a long trip and want to start with 95% at 6am. How do I set the truck/app to nail this target % and time; thus minimizing the amount of time spend at a high SOC?

Right now on normal days I just have it turn on charging at 2am and it goes to my set point… then precondition before leaving at 6am.

My tesla wall charger at 42/48 amps will give me 40% charge back max from midnight to 6 am so back calculate what your normal precondition kwh is based on amps charger is set at to figure it out

Set schedule to stop charging by 6am.

I have found (on our X) if I set a departure time it completes the charge earlier than I want, have seen it finish 45 minutes before the time I tell it. I simply set the SoC to the level I want (say 95% in our example) I then wait to see how long it says to complete. I then do the math and set the charge to start at a time that will get me to the desired SoC within 10-15 (because the smallest time start increment is 15 minutes) of my desired departure time. For example I am leaving for some meetings in the AM and have about 190 miles to cover so I want to leave the house at 80%.

Going to hit the road around 07:00 and it says 3h 30m to reach 80% so I will have it start charging at 03:30

I found this to be the most accurate way for me to not sit at a high SoC for a while.

They still have some work to do on the scheduling, it's possible but could be better!

Scheduling a departure doesn't do anything but pre-condition battery for driving by X time. Currently there is no way to have the computer start charging batter to be "ready" to go by X time. It seems insane that they don't have this. But I've been hoping for months that they do it since the wife uses our 3 to travel weekly. The only way to do this is to back calculate how long it will take to charge from your departure time and schedule charging to start then.

That feature has always been there.

Turn off scheduled charging, set departure time.

And don't worry about going to 100%, it's fine and actually good to do periodically. It allows the battery to top off and the BMS to reset.

All Scheduled departure time does is precondition the battery and start charging immediately to what ever limit you set regardless of if it is too early to time the departure correctly.

Have I been using this feature incorrectly for years?

Yes you have.

You can schedule your charge to finish right at a planned departure time to reduce energy costs, even in market regions where off-peak utility rates are not applicable. Example, if charging starts as soon as you plug in, charging may complete much sooner. This causes the Battery to cool down to ambient temperatures and requires energy to warm it back up by your departure time. Therefore, even if off-peak utility rates are not applicable to you, Tesla recommends that you charge until your planned departure time in order to reduce energy consumption by specifying your departure time as the scheduled End by time.
