What you really need is a little joystick you can push left-right, which translates the motion into the down/up butons. But that would require levers, and/or a more sophisticated attachment.

I 3D printed a toggle switch for the turning signal buttons on a Cybertruck. Because it lacks a turning stalk this helps to emulate one.

I did just have a thought, though … is there a way to wrap it around so that the mechanism works from the back where your grip is and where a turn signal would naturally be? 🤔
