I hope this is being reported as a bug. This hasn't happened to me but could be deadly if it doesn't get resolved.
..agree...hope is it not reported as 'Pedal Confusion'...

Recall Lexus had runaway acceleration reported for over a year....until an off duty Highway Patrolman driving his Lexus could NOT stop the car from accelerating, he was barreling down the fwy at 90+ mph for miles...

10,000 miles, I have not see this. Could is be the collision avoidance intervening due to being close to an object?

I've heard that Tesla touts that the car can determine when you get in - if you need to go FWD or REV on it's own.

This is only entirely possible when it detects something : like you garage wall infront of the vehicle.

...now when it see something in front AND in back...what does it do?

I can definitively tell you in my case there was nothing behind me and there was a guardrail directly in front of me. After it went forward the first time I checked the directional indicator on the screen, still in reverse, and pressed the accelerator again, it went forward.

Shit...that's a real problem.

You know they went all-in on these 'buttons' for Transmission control - and you don't have 'haptic' feedback.

..Back in the day when you shoved the shift level into Reverse - you KNEW with your eyes closed it was in reverse...

Now it's all gentle taps...

Recall we lost Chekov - Actor Aton Yelchin: he was CRUSHED TO DEATH by his jeep...he 'pressed' Park on the gear shift...it did not take the input...the jeep - still in drive - rolled forwarded as he was opening his home gate and crushed him to death..

..So this shit is real..

They need to put a solenoid on the button - that keeps the button out - until the software 'reads' your press - then allows to the button to depress - as physical feed back that your button press input was 'read' by the software.

..I work designing medical Devices, have worked on software control of Neutron Guns, Particle Accelerators and other things that can kill you in creative ways...

I want everyone to get home safe tonight...

I complained about the forward/reverse issue on here a few months back. It would back into my trailer when I had it in drive.

This isn't true. It was always possible to miss the reconnection after navigating the shift plate, although the failure mode was more likely neutral - many people have been crushed by automatic cars in the past. Since they depended upon a fluid drive or a friction drive, the act of getting out of the vehicle would change the rolling resistance and the vehicle would start to move. It's not a new phenomenon.

This happened to me two weeks ago and was like a quarter of an inch from running into something. I put it in reverse, it went forward. I checked it was in reverse, it went forward. I put in park and put back into reverse again and it went in reverse.
This, if I understand your post, happens to me often. I will back-out of the garage and as I place the car in drive, the car automatically changes its position back to reverse. Happens often.
that’s weird.. mine didn’t switch back it was going the wrong way haha

I've occasionally mis-fired on the directional swipe, causing the gesture to be ignored (and therefore stays in the previously-selected direction).

I believe the OP is suggesting that the vehicle was moving in the opposite direction of that indicated in the UI, which is hilarious/horrifying.

Good thing they haven't flipped left-and-right on the SbW yet lol
Correct. Thought it was user error but it was definitely the truck haha.

Alright so this happened to me today for the first time:

Pulled up to a car wash, there’s a wide parking spot with a red pole to block the end, vacuumed the car myself and got back in. Put it in reverse and very cautiously looked around to make sure no one was behind me and I felt the car move forward. Immediately I start thinking “You moron you put it in forward drive”. Reached to switch it to reverse only to see that it already was.. gave it another tiny tap and once again the truck moved forward. Pulled out my phone to record it but it was such a small distance to show the example you can barely tell. Was getting really close to the red pillar in front of the truck so I couldn’t keep on the pedal.

Shut the truck off and booted it back up and it continued to do it. For a moment I was thinking of calling service for help but I put it in forward to see what it would do. Burned up my last inch of space going forward. Put it back in reverse, MOMENT OF TRUTH.. am I going to hit the poll in front of me or will it finally back up? With the tiniest of cautious pressure from my big toe, JUST like the scene from SpongeBob, the truck moves backwards and saves me embarrassment and a heart attack.

Sorry for the long story, just one of those comedic situations but I wanted to share the example in case anyone else is experiencing this bug?

If something like this happens again, use the overhead shift.
Funny you mention that.. in the video I recorded but didn’t post, that’s one of the things I tried checking. Probably did it wrong though, slid my finger instead of pressing at the top so none of the shift options showed up and I accidentally triggered the hazards.

IMO: Tesla should NOT have abandoned LIDAR in lieu of vision only based navigation. All you care about when moving a vehicle is the distance from things around that.

Cameras do not determine distance - you can by interpolation using stereo cameras 'derive' distance...but that is not the same a knowing the distance directly.
